AIM: Postoperative ileus has been considered an inevitable consequence of abdominal surgery. The aim of the study was to investigate the efficacy of same treatments in resolving postoperative ileus in various surgical approaches. METHODS: A total of 360 patients underwent abdominal surgery, and was divided into four groups: videolaparoscopic cholecystectomy, laparotomic colo-rectal surgery, laparotomic Hartmann procedure, laparotomic gastric surgery. The authors present a case of gallstone intermittent ileus caused by the passage of a big gallstone (about 4 cm in diameter) in the intestinal lumen, through a cholecystoduodenal fistula. They emphasize the peculiarity of the case for the characteristics of symptoms and for casual diagnostic check-up with a ultrasonography. The disease is not frequently diagnosed; today it has a safe recognition by modern imaging. The symptoms can be intermittent and, even when there are the classic signs of intestinal occlusion, the site of the occlusion is various. With a timely endoscopical or surgical approach (open or laparoscopic) it is possible to reduce mortality of patients treated in emergency.
Post-operative ileus in elderly patients

Materials and methods Data collected from 20 patients undergoing right hemicolectomy for right bowel cancer were evaluated. Patients included were divided in two groups of 10 subjects respectively, according their age. In the first group were allocated patients of age ≥ 70 years, whereas in the second group were included subjects of age ≤ 60 years. Patients affected by postoperative delirium or presenting complications that could altered the regain of intestinal motility were excluded. The time of first passage of flatus, the time of first defecation and the postoperative stay were evaluated.
